- A pre-enrolment interview.
- Completed International Student Enrolment Form, signed and dated.
- Proof of Guardianship/Custodian Agreement.
- Canadian address and contact information.
- A translated copy of Current Immunization Record.
- A recent photograph of your child.
- Disclosure of any medical conditions, which may arise during the course of the school day, including allergies and medications currently being taken.
- A translated copy of your child’s Birth Certificate and Passport, and supporting documents for study in Canada.
- A translated copy of your child’s report cards covering the most recent year of study.
- Payment of all related fees (tuition, registration, lunch, ESL).
A student is considered accepted into the School upon written confirmation from the School. Century Montessori Private Schools reserve the right to provide or withdraw its service conditional on review of your application.
